A medication error is often defined as any preventable error that initiates the improper use of medications. In 2006, the Institute of Medicine report, Preventing Medication Errors, estimated that at least 1.5 million patients are harmed each year by medication errors. Error prevention is a common concern that affects all patients and healthcare professionals including pharmacists and pharmacy technicians. These errors often include the misinterpretation of a written prescription or verbal communication, inadequate knowledge of patient’s medical or medication history, or incorrect package labeling.

Kristin Weitzel received her Doctor of Pharmacy from the University of Florida in 1998 and completed a Community Pharmacy Practice Specialty Residency with the Virginia Commonwealth University/Medical College of Virginia and Ukrop’s Pharmacies. Dr. Weitzel is currently Clinical Associate Professor and Director of Experiential Education in the Department of Pharmacotherapy and Translational Research at the University of Florida College of Pharmacy and practices in the ambulatory care setting. Dr. Weitzel’s practice and research interests include the treatment of diabetes and depression in ambulatory care patients and innovation in preceptor development and clinical teaching. Dr. Weitzel is a Certified Diabetes Educator and a Fellow of the American Pharmacists Association. |
